Patient received HPV and Flu vaccine. Roughly 7 minutes after administration, RN heard patient's mom call her name and immediately ran to room. Per mom, patient stood up to get a tissue and while walking back to exam table, lost consciousness, hit wall with hands/head, and fell to the floor. LOC less than 5 second. While patient was lying on floor, RN elevated legs and took vitals, patient was awake and speaking. Vitals were WNLs. POC Glucose performed and WNL. Dr. examined patient while lying on the ground. No indication of injury. Assessment normal. Vitals were repeated after about 5 mins and were again WNL. Patient remained lying on floor for 15 mins. RN repeated vitals. WNL. RN assisted patient to chair where patient had a juice box and sat for 5 mins. Patient reported feeling normal. RN assisted patient to standing and waited a minute to assess. Patient reported feeling back to normal. Mom and patient left without assistance.
