Symptom narrative

Pt was seen in office today 4/24/26 for his 2 month Well Visit. A clinical staff member administered the Rotarix vaccine in error, without realizing that the pt received a dose on 4/6/2026. The clinical staff member that administered the vaccine had reviewed the error with the MOC, at the time of administration in office .
