Symptom narrative

Patient states he noticed a red raised rash around injection site (left arm), up his neck, across the top of his chest over to the right arm. States the rash itches. Patient did not report to his parents until Monday. This office was not notified until today 4/24/26. Red rash was noted to left and right arm, upper chest and neck. The rash is greater on the right side than the left. States it is come better today but still itches. Patient was seen by family doctor Monday and was given a Medrol dose pack. Dr diagnosed patient with atopic dermatitis. Patient has have MCV4 vaccine in the past with no adverse effects.
