Per discharge summary dated 04/16/2026, 28 y/o F w/ h/o PCOS on OCP, HTN, and asthma who presented to Medical Center for progressively worsening rash over the area where she received varicella and HPV vaccines 2 days prior. Vaccine reaction Rash Time from point of exposure to the vaccines to onset of symptoms inconsistent with an acute anaphylactic reaction. Patient did not require epinephrine for improvement of symptoms. However, patient symptoms and wax and wane pattern with localized rash with cold like symptoms are suspicious for drug reaction from vaccination. Patient's presentation of afebrile, non-vesicular localized rash without neutropenia is inconsistent with varicella breakout. Symptomatic management with topical steroid, clobetasol with diphenhydramine and APAP prescribed on discharge. Furthermore, patient was discharged with two days of prednisone 40mg once daily. Patient was also counseled on return precautions and concerning findings of rash pertinent to varicella. VACCINE #1: Varicella, For lot number and expiration date see entry in Immunization module.
