Symptom narrative

on 4/25/26 I did a Virtual Urgent Care Visit and instructed to go to the ER. I went to Er apprx 10am. Tests were done. CRP, CBC, Comprehensive Metabolic Panel, Electrocardiogram, XR left shoulder 2 or more views. normal soreness at injection site on 4/9 and 4/10/26 went away. Extreme pain returned 4/23/26 and could not use arm or sleep due to sever pain. ice and Tylenol did not help. Sent home from ER with Frozen Shoulder dx 4/25/26 with referral to Orthopedic
