Symptom narrative

Patient reported pain and lump in right upper arm at injection site that develeoped approximately 1 week after receiving the covid vaccine 11/13/25. Patient reported the Covid vaccine was given in that arm. Documentation differs that influenza was administered in the right deltoid. Patient reported first reported the pain and lump 12/8/25 via triage and was seen in the office for evaluation that same day. Warm compresses and tylenol recommended. Ultrasound of the extremity ordered and completed on 12/12/25 that revealed possible hematoma. Patient saw Plastics on 12/17/26- unable to aspirate. No sign of infection. Ordered continue acetaminophen or ibuprofen, range of motion, repeat ultrasound in one month-completed on 1/20/26 revealed possible hematoma. 4/14/26- right shoulder mass excision completed by Plastics. Patient also under the care of orthopedic for unrelated right rotator cuff issues, knee and joint issues requiring physical therapy.

Genetic testing . Treatment for weight loss.

ocular hypertension b/l, open angle b/l, esophageal reflux, tonsilar hypertrophy, anisocoria, hypothyroidism, cystoid macular edema left eye, IFG, OSA, hypertension, foreign body left cornea, hx of corneal transplant, Haglunds deformity right, bulous keratopathy, osteoarthritis, hyperlipidemia, depressive disorder, edema, palpitations, morbid obesity, atypical ductal hyperplesia breast

amlodipine, aspirin, calcium, magnesium, citalopram, latanoprost gtt, levothyroxine, losartan, omega-3 fatty acid, prednisolone gtt, semaglutide, timolol gtt
