Site: Pain at Injection Site-Severe, Site: Redness at Injection Site-Severe, Site: Swelling at Injection Site-Severe, Systemic: low grade fever - pt reported all of above symptoms via phone to have after 2 hours later of vaccination-Medium, Additional Details: pt called store(?) - I was left with a note to follow up with pt on monday 4/20/2026. Pt called store on saturday 4/18/2026. Pt stated that she had redness and swelling. Rph on duty did not report this on saturday 4/18/26. I, called pt on 4/20/2026, spoke with pt, pt is doing well. She reported that she had swelling, redness, and low grade fever after 2 hours of vaccination. It was extremly painful when injection was given. Pt was informed to take ibuprofen and use ice to eleviate pai
