received the adjuvants only for Arexvy; received the adjuvants only for Arexvy; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of inappropriate preparation of medication in a elderly male patient who received RSVPreF3 adjuvanted (Arexvy) for prophylaxis. On an unknown date, the patient received Arexvy. On an unknown date, an unknown time after receiving Arexvy, the patient experienced inappropriate preparation of medication (Verbatim: received the adjuvants only for Arexvy) and inappropriate dose of vaccine administered (Verbatim: received the adjuvants only for Arexvy). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK receipt date 17-APR-2026 A call was received today stating that one resident in a long term care facility may have received the adjuvant only for Arexvy due to a possible medication error. The healthcare professional stated that there was no way to determine whether the medication was correctly administered or if there was a medication error. The batch number was not provided, and we are unable to contact the reporter.
