Symptom narrative

Patient was supposed to get Hep A, MMR, and Varicella but was inadvertently given MMRV (ProQuad) instead of MMR. MMRV had been stored in the refrigerator. Admin error reported to Merck Medical team to get additional information on viability of MMR component of MMRV stored in the refrigerator. Awaiting that information.
