Incorrect Preparation; Incorrect Preparation; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 18-year-old female patient who received Men ABCWY NVS (Penmenvy) (batch number 5S723, expiry date 08-OCT-2026) for prophylaxis. On 15-APR-2026, the patient received the 1st dose of Penmenvy. On 15-APR-2026, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: Incorrect Preparation) and inappropriate dose of vaccine administered (Verbatim: Incorrect Preparation). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 17-APR-2026 The nurse reported that the patient that came in for the Penmenvy vaccination and they received only the syringe part and not the vial part which vial was the Menveo part. The reporter need to know what to did for the patient.
