Symptom narrative

SYMPTOMS: Had Physical exam 4/14 and received pneumonia vaccine. That night had episode of vertigo. Had another episode 4/15 after feeling fine all day, but woke with feeling dizzy and nauseous 4/16. Lasted about 90 minutes. Did not feel she could drive to work so stayed home. These intermittent symptoms have continued, patient was seen in office yesterday 4/27 with ongoing symptoms. Vaccine site is not sore to touch, warm, or red.

4/3 seen in urgent care for abdominal pain

Arthritis, migraine, obesity, osteoporosis/osteopenia
