Symptom narrative

Pt. c/o right arm redness, swelling, pain, hot to touch, rash without raised bumps. Went to facility, HCP gave her dose of Benadryl and a prednisone steroid pack for a 5 day course. Spoke to pt. on 4/27/26, stated her arm was much better. Asked about left arm, and pt. stated it was red and sore but not like right arm. Pt. called HD on 4/24/26. Secretary took note and called RN to inform of pt. complaint. RN was out of town at a conference and after hearing from secretary, I decided to call pt. on Monday morning. 4-27-26 - called pt. at 0930 on Monday morning. Asked pt. what went on and pt. described intense pain noted 6/7 out of 10, redness, swelling, hot to touch, and a rash with no bumps at site of injection on right deltoid (shots given on 4/21/26 at 4pm). Stated left arm injection site was red and sore but nothing like her right deltoid. Pt. stated she went to facility on 4/23/26. HCP there gave her benadryl PO and a five day course of prednisone. Pt. stated that her arm was much better at this time.
