Symptom narrative

Patient came to the clinic for wellness child visit with Dr. Based on immunization registry, she needs vaccine for Hep A, D tap /TD/Tdap, IPV and Hep B, patient had only received 2 doses prior to this visit. Dr. ordered the vaccine as recommended in CDC age appropriate vaccine. Pt has an order for Dtap/HepB/IPV(Pediarix) and Hep A( Havrix) was verified with MA and was given by me as ordered on right arm Pediarix and Hep A on left arm and was discharged without reaction with father. Apparently, pediarix is for use from 6 weeks through 6 years of age and pt is 7 yo.
