Symptom narrative

Patient called RN phone stating that the morning after receiving Tdap vaccination she developed a blister on the arm that she received Tdap in. She is not sure if it is located at the injection site. Size of thumb, filled with fluid (not pus). She reported an initial red ring around blister that had subsided. Soreness related to Tdap vaccine reported without any other signs/symptoms of infection. No fever reported, not hot to the touch. Referred her to Occupational Health to be evaluated by a Provider to rule out localized infection.
