Symptom narrative

Patient received PCV20 vaccine in the right deltoid on 4/13/26 during PCP visit. Same day onset of right arm/shoulder pain with mild swelling and warmth at injection site. On 4/15/26, patient reported persistent pain, swelling, and warmth without improvement or worsening; denied fever, spreading erythema, drainage, dyspnea, hives, or facial/throat swelling. Home care and return to clinic instructions provided by RN. At in person visit on 4/23/26, patient reported ongoing localized right shoulder pain with tenderness, palpable lump/knot at injection site, warmth, and decreased range of motion with difficulty lifting arm. Denied issues related to prior right shoulder arthroplasty (~3 years ago) and denied signs of joint infection. Advised to continue lidocaine patches, apply ice, consider otc pain relievers, perform light range of motion exercises, and monitor symptoms.
