Patient received PCV 21 at office visit 4/24/26 at 2:05 pm. Patient reported on Saturday she woke with pain in R arm and could not "use my right arm not to comb hair or put on clothes." Pain reported 8 or 9. Patient attempted to alleviate symptoms with ice and then a heating pad and Tylenol. Patient attempted to reach pulmonology office and then contacted PCP office. Patient was seen in PCP office on 4/27. Patient reported to PCP after injection she immediately developed a sharp pain at the injection site with an "electric shock-like" sensation radiating down the arm. By that night she developed marked functional limitation with inability to raise or abduct the arm beyond a minimal range. Patient was diagnosed by provider with shoulder injury related to vaccine administration (SIRVA) with adhesive capsulitis. Patient was prescribed oral steroids. Plan for follow-up on 4/29/26 and if no improvement in symptoms, MRI will be ordered to further assess.
