Symptom narrative

Approximately 20 minutes after receiving varicella vaccine, developed diffuse pruritus, throat tightness, mouth tingling, rash/body burning sensation. Evaluated in emergency department. Treated with IM epinephrine, IV diphenhydramine, famotidine, and methylprednisolone. Observed 4 hours. Final diagnosis documented as anaphylaxis
