Kinrix instead of Pediarix administered; Kinrix instead of Pediarix administered; This non-serious case was reported by a other health professional via call center representative and described the occurrence of wrong vaccine administered in a 5-month-old female patient who received DTPa-IPV (Kinrix) (batch number n7gc9, expiry date 19-OCT-2026) for prophylaxis. Co-suspect products included DTPa-HBV-IPV (Pediarix) for prophylaxis. On 20-FEB-2026, the patient received Kinrix. The patient did not receive Pediarix. On 20-FEB-2026, an unknown time after receiving Kinrix and the patient experienced wrong vaccine administered (Verbatim: Kinrix instead of Pediarix administered) and inappropriate age at vaccine administration (Verbatim: Kinrix instead of Pediarix administered). The outcome of the wrong vaccine administered and inappropriate age at vaccine administration were not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 20-APR-2026 The reporter reported that a nurse administered Kinrix instead of Pediarix to a 5 month 8 day year old child on 20-FEB-2026. This dose should have been the patient's 2nd dose of Pediarix. The batch number was not provided upon follow- up with the reporter.
