only liquid component administered; only liquid component administered; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a 11-year-old male patient who received Men ACWY-CRM NVS (Menveo) (batch number AMVB079A, expiry date 31-MAY-2026) for prophylaxis. On 21-APR-2026, the patient received Menveo. On 21-APR-2026, an unknown time after receiving Menveo, the patient experienced inappropriate preparation of medication (Verbatim: only liquid component administered) and inappropriate dose of vaccine administered (Verbatim: only liquid component administered). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK Receipt Date: 21-APR-2026 The reporter reported that the patient received accidentally only the liquid component of Menveo (2-vial) without being properly reconstituted with the powder.
