may have received expired dose; This non-serious case was reported by a other health professional via call center representative and described the occurrence of expired vaccine used in a 1-month-old female patient who received DTPa-HBV-IPV (Pediarix) (batch number 77J49, expiry date 09-FEB-2024) for prophylaxis. On 15-AUG-2024, the patient received Pediarix. On 15-AUG-2024, an unknown time after receiving Pediarix, the patient experienced expired vaccine used (Verbatim: may have received expired dose). The outcome of the expired vaccine used was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK receipt date: 21-APR-2026 The reporter had a patient who had received their two year vaccination. When it was scanned into the patient's chart in 2024, the lot's expiration date showed 09 FEB 2024. The reporter thought that the patient had received an expired vaccination and that the record had been scanned rather than manually entered into the computer.
