Symptom narrative

vaccine was inadvertently administered to a patient that was 14.5 weeks pregnant; No symptomatic events were reported; This spontaneous prospective pregnancy report was received from a nurse and refers to a 21-year-old female patient. The patient's medical history was not reported. Concurrent conditions included sickle-cell trait. Concomitant medications included ondansetron (ZOFRAN) and minerals nos (+) vitamins nos (PRENATAL VITAMINS). She had no drug reactions or allergies. On an unknown pregnancy conception date (estimated as approximately 26-JAN-2026), the patient became pregnant, with last menstrual period (LMP) date estimated as approximately 12-JAN-2026. This pregnancy involved 1 fetus (by default). Pregnancy due date was not reported but calculated as approximately 19-OCT-2026. On an unspecified date in March 2026, the patient underwent obstetric laboratory tests (reported as "OB-labs"; results not reported). On 24-APR-2026, the 14.5-week-pregnant patient was inadvertently vaccinated with 1st dose of human papillomavirus 9-valent vaccine, recombinant (GARDASIL 9) suspension for injection at a total dose of 0.5 mL, administered intramuscularly, lot #A003663, expiration date: 11-DEC-2026, for prophylaxis (anatomical location was not reported) (Exposure during pregnancy).
