Symptom narrative

PCV 20 vaccine was administered in the lower part of the upper arm, right above the elbow., not in the deltoid on 4/23/26. Shingrix vaccine was administered on the back of the left arm in subcutaneous tissue. Patient had a follow up appointment on 4/28/26, due to vaccine sites being red, warm to touch and a rash present. Patient was prescribed and antibiotic for the cellulitis and an antihistamine for itching.
