Symptom narrative

Pt had a brief syncopal episode (approx. 20 seconds) after administration of first dose of HPV vaccine; pt said she felt like she was going to 'pass out' and slumped forward towards this (administering) nurse. Pt was laid down in a safe location, given cool compresses for her neck, and cool water to drink when she regained consciousness; the temperature in the room was also cooled down/ door opened, as it was very warm. Vital signs were stable. Pt was monitored for 15 minutes post-syncopal episode and no other complications arose.
