Symptom narrative

Pt contacted the clinic on 4/16/26 stating the vaccine went into her 'joint, not my muscle' and that she had lost range of motion to her joint and it was painful. The provider (FNP) had the patient come back in for an evaluation, she assessed the site and there was no redness, but she had tenderness at the sight. She was treated accordingly and advised to follow up as needed. The patient contacted the clinic on 4/25/26 stating that she is still experiencing shoulder pain and causing her difficulty with everyday task. The FNP has now requested to order an EMG.
