Symptom narrative

Dtap and Hib vaccines were given to patient, who tolerated vaccine administration well with no reactions and no injection site swelling or pain. On reviewing patient chart after discharge, noted that Dtap and Hib vaccines were given on 3/6/2026 and 4/8/2026 and both instances these vaccines failed to transcribe onto state database due to an EMR transcription error. Patient had missed/no-showed to multiple routine appointments which led to clinic staff assuming patient is behind on vaccinations and provided the vaccinations that were appearing due per state database. Confirmed with parent that patient is asymptomatic and doing well.
