The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 14-Apr-2026, the patient was vaccinated with a dose of Measles, Mumps, and Rubella (Wistar RA 27-3)(M-M-R II) Virus Vaccine, Live (lot #Y013130, expiration date: 30-Jul-2026) (.5 ml) (anatomical location and administration route were not reported); diluted with an improperly storage dose of sterile diluent (Hollister-Stier STERILE DILUENT), (lot #X009406, expiration date: 23-Nov-2025) as prophylaxis. The reporting pharmacist called to report an expired dose of the sterile diluent (Hollister-Stier STERILE DILUENT) that was inadvertently administered to a patient, along with Measles, Mumps, and Rubella (Wistar RA 27-3)(M-M-R II) Virus that was not expired.
