Previously administered products included for Product used for unknown indication: COVID-19 vaccine (Dose 1). Past adverse reactions to the above products included No adverse effect with COVID-19 vaccine. On 13-Jul-2021 at 6:27 PM, the patient received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) 1 dosage form. On 13-Jul-2021, the patient experienced VACCINATION SITE MASS (lumps in his shoulder/lumps forming at the injection site). On an unknown date, the patient experienced DISCOMFORT (The lumps are flat and they are causing discomfort).
