Symptom narrative

He reports the evening after getting the vaccine he began to get very "itchy". It has not stopped yet. It has been 17 days. Sent to both clinics here in town to see if the can see him "today". Referred to clinic as he has no PCP. Appt. set x 4/21/26 @ 1pm. Appt. 4/21/26 - seen by APRN. Rx'd 1. Loratidine 10mg daily x30 days. 2. Kenolog IM 40mg x1. 3. Steroid taper (PrediPack) x1. (He does not have a flu appt. scheduled unless rash doesn't resolve. 4/27/26). 4/29/26 2:39pm - I called him to see if the rash & itching had resolved. He reports it worked well & he is feeling "perfect" now. Case closed.
