Symptom narrative

2 days after receiving both vaccines he began to experience significant pain, which radiated to the left side of the neck, and by day 5 after vaccination he went to urgent care where he received a toradol inj and a Rx for muscle relaxant. he continued to have appreciable but tolerable pain/spasm until he had massage therapy about 2 weeks after vaccination. He was seen in clinic 6 weeks after the vaccination (he was traveling out of state so we were unable to evaluate him until today), when he had a normal deltoid and neck exam, including FROM. He has not taken any NSAID or muscle relaxant for > 2 weeks.. Note, he also received influenza and HBV vaccines in the right arm/delt, without any local site reaction.
