Symptom narrative

Urticaria on posterior trunk. Client complains it is very itchy and hurts fist noticed after a bath on 4/29/26. Client went to the Emergency room the morning of 4/30 as urticaria had spread and client was crying and saying it itched. Caregiver had given Benadryl on 4/29/26 and applied anti-itch cream per caregiver. Caregiver did state the client had gone on a field trip on the day the urticaria was discovered.

Anxiety medication possibly fluoxetine, hydroxyzine, trazadone, melatonin all nightly to help sleep
