On Monday, April 27, a temperature excursion occurred when the vaccine refrigerator went out of range overnight. The unit returned to the appropriate temperature range prior to patient visits. However, a patient had already been vaccinated before final guidance was received from the manufacturers. All affected vaccine manufacturers were contacted, and based on their guidance, the vaccines involved in the excursion were determined not to be compliant for use. The provider was notified, the patient?s family was contacted, and the patient will need to be revaccinated per manufacturer and clinical guidance.
