Symptom narrative

Per patient and parent, reports of left arm pain and swelling that began after receiving vaccines on Tuesday. The arm alternates between feeling cold and hot, and swelling that has extended down to his hand- at times preventing finger movement. Patient has shown decreased interest in activities. Patient has developed a rash under the left arm, which was not present prior to the injection. Per provider visit today, the rash is not itchy, patient reports that left hand and arm were clammy yesterday and it was difficult to touch due to discomfort. Increased sweating of the affected arm when indoors.

History of hypoplastic left heart syndrome, status post heart transplant, history of severe dehydration. Hx multiple heart surgeries

Iron, sirolimus, envarsus, pravastatin, baby aspirin, xyzal, flovent, magnesium

NSAIDs, Adhesive, diphenhydramine HCl, fluorouracil-adhesive bandage, latex, natural rubber
