Symptom narrative

After Gardasil injection was given, the patient received a lab drawn as part of his routine well check. Several minutes later, the patient was asked if he felt light headed or dizzy, his response was no. Once he stood up, he experienced a syncopal episode. This resulted in him falling face forward and hitting the back of the door. He lost consciousness for roughly 60 seconds. The fall resulted in a fracture of the mandible requiring multiple surgeries to repair.
