pt presented today and let us know that last night she came to our pharmacy and requested a meningococcal A shot for college/dorm purposes. we had processed and administered bexsero (men B) and pt realized it after the fact. pt had no adverse reactions. i'm filling this out to report the fact that we processed and administered the wrong form of meningococal vaccine to pt.
