Second dose of vaccine administered to patient expired on 04/30/2026, approximately 12 hours before administration to patient. No adverse effects noted post administration. Pending recommendation from patient's PCP, a third dose may be administered. Manufacturer was contacted. Store leadership notified and error documented in store pharmacy system. Patient notified. All notifications took place the same day the expired vaccine was administered. No method of determining if any degradation of vaccine potency had occurred in the short time of expiry prior to patient administration.
