Patient came in to request a Shingrix vaccine. According to the vaccine registry this was her 2nd dose. Vaccine was administered. Patient returned 3 days later with redness and warmth at the injection site. Rph on Duty recommended ice, benadryl and cortisone cream. Patient returned to the pharmacy later that day with redness spreading down to elbow. She paged her Dr who then prescribed prednisone. While talking to her Dr the Patient discovered that she previously had 2 shingrix vaccines, one of which was not reported to the registry and that this most recent one was actually her 3rd shot.
