Symptom narrative

Patient stated that the day after he received immunization he "passed out" three times at 3 different places of business. (Not in pharmacy or at pharmacy store location). He said that he fell each time he passed out and that he "woke up" on the floor confused about how he ended up there. He said that the businesses where this happened wanted to call 911 but he refused to allow them. Patient states he has pain in his side and hip presumably from where he fell and he plans to see MD for follow up for that. He states that he does not have any remaining adverse effects he feels from the vaccine at this time and has not passed out since.
