diluted with sterile water; diluted with sterile water; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of wrong solution used in drug reconstitution in a patient who received Herpes zoster (Shingrix) for prophylaxis. On an unknown date, the patient received Shingrix. On an unknown date, an unknown time after receiving Shingrix, the patient experienced wrong solution used in drug reconstitution (Verbatim: diluted with sterile water) and inappropriate dose of vaccine administered (Verbatim: diluted with sterile water). The outcome of the wrong solution used in drug reconstitution and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK Receipt Date: 23-APR-2026 Shingrix was diluted with sterile water and was given to patient. Colleague had read the labelled information about diluent usually used and no information was available. The pharmacist called to enquire about the next step. The batch number was not provided upon follow- up with the reporter.
