On an unknown date, the patient received Shingrix. On an unknown date, an unknown time after receiving Shingrix, the patient experienced wrong solution used in drug reconstitution (Verbatim: diluted with sterile water) and inappropriate dose of vaccine administered (Verbatim: diluted with sterile water).
