Symptom narrative

he has not yet received the second dose; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of incomplete course of vaccination in a 52-year-old male patient who did not receive Herpes zoster (Shingrix) for prophylaxis. Previously administered products included Shingrix (received first dose on 19-NOV-2024 with batch number ga99k and having expiry date 29-OCT-2026). The patient did not receive the 2nd dose of Shingrix. The patient had incomplete course of vaccination (Verbatim: he has not yet received the second dose). The outcome of the incomplete course of vaccination was not applicable. Additional Information: GSK Receipt Date: 27-APR-2026 The pharmacist reported that it had been approximately two years since his patient received his first dose. The patient had forgotten to received the second dose. The patient had not yet receive the second dose. The batch number was not provided and a request for this information has been made.
