The patient did not receive Penmenvy.The patient experienced intercepted medication error (Verbatim: they did not end up giving the vaccine at all), inappropriate preparation of medication (Verbatim: only grab the Men B prefilled syringe component) and inappropriate dose of vaccine administered (Verbatim: only grab the Men B prefilled syringe component). The outcome of the intercepted medication error, inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable.
