they did not end up giving the vaccine at all; only grab the Men B prefilled syringe component; only grab the Men B prefilled syringe component; This non-serious case was reported by a nurse via sales rep and described the occurrence of intercepted medication error in a patient who did not receive Men ABCWY NVS (Penmenvy) for prophylaxis. The patient did not receive Penmenvy.The patient experienced intercepted medication error (Verbatim: they did not end up giving the vaccine at all), inappropriate preparation of medication (Verbatim: only grab the Men B prefilled syringe component) and inappropriate dose of vaccine administered (Verbatim: only grab the Men B prefilled syringe component). The outcome of the intercepted medication error, inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 21-APR-2026 Reporter stated that they were going to administer Penmenvy and only grab the Men B prefilled syringe component, they did not end up giving the vaccine at all because the patient changed their mind, but they got to know that vaccine needs to be reconstituted.
