Symptom narrative

No symptomatic events were reported; Improperly stored vaccines. All dates listed were in 2026 (2/4/2026: Fridge 47.4F 1 hour/28.9F 45 min; 3/13/2026 freezer: 6.88F 2 hours 30 min/ fridge 47.3F 30 min, 35.4F 30 min; and 3/14/2026: fridge 49.6F 1 hours 30 min); This spontaneous report was received from a physician Assistant and refers to a currently 11-years-old female patient. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 06-Feb-2026, the patient was vaccinated with improperly stored doses, reported as temperature excursion of: Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL 9), Suspension for injection, prefilled syringe, Batch/lot number Y013818 has been verified to be a valid batch lot number , expiration date not reported, but upon internal validation established as 12-Feb-2027, administered by Intramuscular route in Left Arm; Diphtheria vaccine toxoid;Pertussis vaccine acellular 3-component;Tetanus vaccine toxoid (BOOSTRIX) (lot #CX4HL, expiration date: 09-Nov-2026) , administered by Intramuscular route in the left arm; and Meningococcal vaccine A/C/Y/W conj (tet tox) (MENQUADFI) (lot #u8600AA, expiration date: 31-Oct-2028), administered by Intramuscular route in the Right arm for prophylaxis, with public funds at private doctor's office (Product storage error).All the doses administered as prophylaxis. The vaccines had temperatures excursion of 47.4 F for 1 hours 0 minutes 0 seconds, 28.9 F for 0 hours 45 minutes 0 seconds, 6.88 F for 2 hours 30 minutes 0 seconds, 47.3 F for 0 hours 30 minutes 0 seconds, 35.4 F for 0 hours 30 minutes 0 seconds and 49.6 F for 1 hours 30 minutes 0 seconds. No adverse event was reported and no symptomatic. This is one of several cases reported by the same reporter.
