Symptom narrative

Patient presents for left leg swelling. Patient had health check yesterday morning, approxemitly 36 hours ago, where he received 2 vaccines. These were Kinrix (DTaP and polio) and MMRV. Mom states both vaccines were given in patient's left thigh, and the documentation from that visit supports this. Mom states she just noticed swelling, and brought patient straight here at 6:12pm 5/2/2026. Mom is unsure when swelling began. Patient is not complaining of any pain or difficulty with range of motion. Patient does admit area is tender to palpation. Denies fever, other trauma, drainage of area, pruritus, warmth, and involvement of hip or knee. 5/3/2026 Patient presents for 24hr f/u on vaccine reaction causing left leg swelling. Mom states swelling has not improved despite warm compresses, cool compresses, and ibuprofen. Patient still denies pain or difficulty with range of motion, but admits to tenderness on palpation. Denies fever, other trauma, drainage of area, pruritus, warmth, and involvement of hip or knee. treatment: Localized swelling of left lower extremity Start prednisoLONE Solution, 15 MG/5ML, 3 mL, Orally, twice a day, 5 days, 30, Refills 0 Notes: Little to no improvement. Since it has been over 48hrs, will initiate steroids to decreased inflamation and pain. No signs of infection at this time. Will reassess again with 24-hour follow-up. Advised mom to continue warm compresses.
