The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 16-Feb-2026, the patient was vaccinated with the first doses of improperly stored Diphtheria and Tetanus Toxoids, and Acellular Pertussis, Inactivated Poliovirus, Hib Conjugate (MPC) and rHBV Vaccine (VAXELIS), (lot #U8313AA, expiration date: 30-Sep-2027) administered by Intramuscular route in Left Thigh for prophylaxis, Rotavirus Vaccine, Live, Oral, Pentavalent (Rotateq), (lot #2142136, expiration date: 28-May-2026), administered by Oral route for prophylaxis and Prevnar 20 (Pneumococcal vaccine conj 20v (CRM197)) (lot #MG7418, expiration date: 30-Sep-2026) administered by Intramuscular route for an unknown indication (Product storage issue).
