I had two brothers in room for well-child exams and sport physicals. Their mother and I discussed whether either child needed vaccines. The mother stated that the 14-year-old likely needed at least one, while the 16-year-old should be up to date. I asked a staff member to print the vaccine records for both boys to verify their need for vaccines. The staff member informed that one of the boys needed a meningococcal immunization. I prepared the vaccine and supplies and took the tray into exam room 8. When I walked into the room the 14-year-old was on the exam table with his sleeve pulled up ready for the vaccine. I prepped the area and administered it. I left the exam room to document the vaccine in his chart. As I was documenting, the other staff asked if I would like her to reprint the vaccine records for mom to have the updated record. The other staff then asked me who I administered the vaccine to. I stated to the 14-year-old. She informed me that the vaccine should have been given to the 16-year-old. I quickly reported to my supervisor and then to the provider. Following this, I contacted the vaccine company GSK for guidance on how to move forward with this issue. Although we recommend administering the Bexsero vaccine to patients at age 16, guidance from GSK indicates that Bexsero is approved for use in individuals aged 10 through 25. This will affect the vaccination schedule, as the patient will need to return in 6 months for second dose to complete series. Follow-up with Parent and 16-year-old is occurring.
