No symptomatic events were reported; Improperly stored vaccines. 2/4/2026: Fridge 47.4F 1 hour/28.9F 45 min; This spontaneous report was received from a medical assistant and referred to an 11 -year-old male patient. The patient's medical history, concurrent condition and past medications were not reported. The patient's concomitant medication included meningococcal vaccine A/C/Y/W conj (tet tox) (MENQUADFI). On 04-Feb-2026, the vaccine human papillomavirus 9-valent vaccine, recombinant (GARDASIL 9) went through temperature excursion of 47.4F for 1 hours 0 minutes 0 seconds and 28.9F for 0 hours 45 minutes 0 seconds. On 06-Feb-2026, the patient was vaccinated with improperly stored first dose of human papillomavirus 9-valent vaccine, recombinant (GARDASIL 9) (lot number: Y020531, expiration date: 05-Aug-2027; formulation, strength and dose were not reported) via intramuscular in left arm for prophylaxis (product storage issue).
