Symptom narrative

No symptomatic events were reported; Improperly stored vaccines. 2/4/2026: Fridge 47.4F 1 hour/28.9F 45 min; This spontaneous report was received from a medical assistant and referred to an 11 -year-old male patient. The patient's medical history, concurrent condition and past medications were not reported. The patient's concomitant medication included meningococcal vaccine A/C/Y/W conj (tet tox) (MENQUADFI). On 04-Feb-2026, the vaccine human papillomavirus 9-valent vaccine, recombinant (GARDASIL 9) went through temperature excursion of 47.4F for 1 hours 0 minutes 0 seconds and 28.9F for 0 hours 45 minutes 0 seconds. On 06-Feb-2026, the patient was vaccinated with improperly stored first dose of human papillomavirus 9-valent vaccine, recombinant (GARDASIL 9) (lot number: Y020531, expiration date: 05-Aug-2027; formulation, strength and dose were not reported) via intramuscular in left arm for prophylaxis (product storage issue). No symptomatic events were reported (no adverse event). This is one of several reports received from the same reporter.
