Symptom narrative

from telephone note to allergist, "On Tuesday, the site became itchy, so she took 2 additional Atarax for a total of 100mg; this was ineffective. On Wednesday, she woke up with the itch escalating to a rash; she took Allegra and 4 Atarax, but by mid afternoon, the rash was worse, so she took an additional 2 Atarax for a total of 150mg. The site appears like a pimple but is mostly flush to the skin. Legs and bottom of feet are also extremely itchy, in addition to reporting stomach upset and headaches. No new foods/products that she's aware of. She explained that because of her work schedule as a caregiver for men with special needs, she is unable to make this Tuesday work for an appointment opening Dr. had. She plans to send images through medical records, but in the meantime, advised patient apply topical steroid cream such as Hydrocortisone to the site."
