Symptom narrative

the patient was administered VARIVAX that has suffered a temperature excursion; No additional AE/ no PQC; This spontaneous report was received from pharmacist and refers to a patient of unknown age and gender. The patient's medical history, current conditions and concomitant therapies were not reported. On an unknown date, the patient was vaccinated with an improper storage dose of Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX), (lot #Z016561, who had been verified to be an valid lot #, expiration date reported and validated as 28-OCT-2027) 0.5 ml/ series (exact dose, anatomical route of administration and frequency were not reported) as prophylaxis; the vaccine was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT). The vaccine was storage at temperature 12F, with a time frame 2 hours 30 minutes 0 seconds. There was no information about previous temperature excursion (product storage issue). No adverse events reported.
