Symptom narrative

On 4/13/2026 client woke up with a rash that was slightly raised and non-itchy that started on his head and neck three weeks after his vaccination. He stated that he also has a swollen lymph node behind his left ear. He described a headache that lasted 36-40 hours. He was not sure if he had a fever. The rash started spreading from his head to his trunk and extremities over the course of a few hours on the first day. The rash lasted a total of four days. He was seen at a local urgent care by a medical provider. He stated that he mentioned that he had eaten fish the day before and the medical provider seemed to think that was the cause of his rash.
