Hiberix liquid portion only administration (potential); Inappropriate dose of vaccine administered; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a 4-month-old male patient who received Hib (Hiberix) (batch number C47XX, expiry date 20-OCT-2026) for prophylaxis. On 26-FEB-2026, the patient received Hiberix. On 26-FEB-2026, an unknown time after receiving Hiberix, the patient experienced inappropriate preparation of medication (Verbatim: Hiberix liquid portion only administration (potential)) and inappropriate dose of vaccine administered (Verbatim: Inappropriate dose of vaccine administered). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable.
