Symptom narrative

Unwitnessed syncopal episode. Staff heard noise, upon entry to room patient was found lying on left side, dazed, appearing pale and diaphoretic. Initial Blood pressure 87/52 pulse 47. Blood Sugar 76. Pt alert and oriented x3, reported feeling unwell and dizzy.Pt was provided ice pack, water and juice. During recovery, pt reported a transient buzzing in his ears, dizziness had resolved, expressed some mild neck discomfort. Vital signs improved and remained stable throughout recovery period. Pt admitted to feeling dizzy soon after vaccination and did not press the call button that was within his reach before fainting. Pt educated to advised health professionals of his hx of syncope as this was his second at SHC.
